Anonymous Tip Form — Use this form to provide the Ferguson Township Police Department with information about suspected criminal activity. You are not required to include any personal or identifying information in the form.

Child Safety Seat Inspection and Installation Form — The Ferguson Township Police have certified officers who are trained to properly inspect and install child safety seats. To schedule an appointment for a child safety seat inspection and installation, please use this form. Please specify in the questions/comments box how many seats need to be inspected. Parking Ticket Appeal Request — Submit a request to appeal a parking ticket within 10 days of the violation.
Residential Emergency Response Notification Form — Please use this form and submit to Centre County 9-1-1 if you would like to provide confidential information to be conveyed to emergency responders, such as someone in your home who has special needs, advanced medical conditions, or needs assistance in the event of an emergency or evacuation.
Special Events Application — Please complete this application if you or your group are interested in holding a special event in the Township. View the Ferguson Township Special Events Policy. For Large Group Events in a Ferguson Township Park (where attendance will or may exceed 100 people), view the Centre Region Parks and Recreation Large Group Events Policy.
